BMI bands (WHO + RACGP): Underweight <18.5, Healthy 18.5–24.9, Overweight 25–29.9, Class I obesity 30–34.9, Class II 35–39.9, Class III ≥40.
Pathway selection logic (RACGP 2024 + ANZMOSS): Diet/lifestyle alone is first-line for BMI 25–29.9 without comorbidities. GLP-1 receptor agonists (semaglutide, tirzepatide, liraglutide) appropriate for BMI ≥27 with comorbidities or BMI ≥30 without. Bariatric surgery indicated for BMI ≥40, OR BMI ≥35 with type 2 diabetes / sleep apnoea / hypertension / fatty liver, OR BMI ≥30 with poorly-controlled type 2 diabetes after failed medical management. PBS-subsidised semaglutide (Ozempic) requires type 2 diabetes diagnosis + HbA1c thresholds — Wegovy + Mounjaro are NOT on the PBS for weight loss as at 1 January 2026.
Cost references: Telehealth GLP-1 (Juniper, Mosh, Pilot, Compass): $290–$450/month subscription (consult + script + admin bundled). Medical/GP-led GLP-1: $130–$500/month script + $250–$800 one-off program fee. Multidisciplinary program: $3,000–$8,000 over 6–12 months. Bariatric surgery (out-of-pocket after Medicare + private health fund): sleeve gastrectomy $13,000–$25,000, Roux-en-Y bypass $18,000–$30,000, lap-band $10,000–$18,000, revision $20,000–$35,000.
